A roster must be filled out before any traveling team can play at our facility. Please print off the form below and mail it to us at PO Box 47021, Indianapolis, IN 46247 or bring to the facility on the night of your first game. If you would like a hard copy call us at 784-7447 and we can mail it to you. We must have both sides filled out completely before we will accept the form. This includes addresses, signatures, parent signatures, etc. |
